Fix the source relationship, then retest

October 19, 2026

A warning is not a request to change a zero count to one. Nor is it a reason to delete a median just to make a check pass. A repair needs to explain how the underlying history and the export should relate.

For applicable charges, compare the count and statistics with the remittance source. If there are qualifying remittances, calculate the statistics and count from that history. If there are none, apply CMS's no-claims treatment and explanation. If the charge is fully calculable as a dollar amount, verify that the encoding reflects that case.

Retain the original row, the applicability decision, the evidence behind the change and the retest result. That creates a record of a source-backed repair rather than a cleaner-looking file whose numbers cannot be explained.
